Before Part 2
Understand the scope and readiness profile
Required first assessment for every customer: Level 1A Access for one named site or function at $1,500 prepaid. You may identify wider or deeper needs here, but those choices are not add-ons. After the qualified Level 1A reports, Salus personnel will discuss and separately price any additional work with you.
Part 2 carries forward your nine Part 1 answers. After sign-in and consent, it asks 19 shared readiness questions and, when relevant, a few questions tailored to your organization. It then asks about the operation you want covered and your priorities. You can save and return. Your answers help Salus prepare a preliminary Level 1A starting scope and identify possible later needs; Part 2 is not a completed assessment.
You do not need to conduct research or obtain new records before continuing. Answer truthfully from your current knowledge. When you do not know, select Not sure or Prefer to discuss with Salus where available rather than guessing.
Part 2 Account
Create an account or sign in
Your Part 2 account protects the confidential organization profile and lets you save your work, sign out, and return later. Submitted results remain available through your account while the record is retained. A 24-hour verification link or browser session is only a security credential; it does not delete or expire your assessment results.

Part 2 — Confidential Organization and Service Profile
Help us prepare your tailored preliminary package
These candid questions help Salus understand scale, complexity, priorities, timing, and likely services. Identifying information is requested only because you elected to continue.
Everyone begins with one Level 1A Access assessment. It covers one named site or function for $1,500 prepaid. For a very small organization whose relevant work fits that single unit, Level 1A may assess nearly the whole operation at source-review depth. For a larger or more complex organization, it is an initial assessment and leadership decision paper. Choices marked as later work are recorded for discussion after your qualified Level 1A reports; they do not increase this order or appear as cart add-ons.
Protected Member and project code names
Create code names that do not disclose the Protected Member’s legal identity, a person’s name, an email address, or sensitive operational details. These names will appear on Part 2 results and remain in effect until an authorized account user changes them. The immutable report reference will not change.
Selecting a government or regulated profile does not itself designate the report as CUI or another controlled category. Salus must review the stated authority before controlled markings are applied.
1. Your organization
These descriptions help Salus tailor later questions and records. They do not set a risk rating, package band or price.
These answers may suggest wider Level 1B/C work later. Every new assessment starts with one named Level 1A site or function. The first Level 1A Access unit is $1,500 prepaid. Other selections do not change that price; Salus discusses and prices additional work after the qualified Level 1A reports.
The $1,500 Access assessment covers one named site or function only. Other requested work is retained for a later, separately scoped decision.
2. Operations and continuity
3. What you need from Salus
Select every outcome you want us to consider. Selection records a need; it does not add work to the $1,500 Level 1A Access assessment. Work beyond its one-unit source review is discussed and separately priced only after the qualified Level 1A reports.
If either role is unknown, you may finish Part 2. The receipt will show a task to assign that role. Salus confirms decision and disclosure authority before agreement or file transfer.
Do not enter passwords, financial account information, medical records, government identifiers, classified information, detailed hazardous-material inventories, access-control details, or information whose disclosure could create an immediate security risk.
Changes will be saved to your Part 2 account.
Optional: share your draft scope for Level 1 review
Only do this if you want an authorized decision owner to request a Salus package review. Sharing a code does not buy an assessment or approve a price.
